WAN : Matmin , have you write a report about yesterday malfunction forklift ?
MATMIN : Yes , I wrote it and e-mailed it to you .
WAN : What incident actually happen ?
MATMIN : Zairi drove the number 7 blue forklift lifting the heavy material when the forklift hydraulic fluid accidentally leak .
WAN : I heard that Babu the operator got minor injury because slipped when walked on the leaked hydraulic fluid .
MATMIN : Yes , he was the one . I rushed to the hospital and the doctor said it was a minor injury and gave him 2 day medical leave .
WAN : So , was the forklift repaired ? Who repaired it ?
MATMIN : Yes , by the evening before the shift change , Ali the maintenance crew completed the repair .
WAN : After this , make sure there is no more similar incidents . Remind all operator to practice safety first all the time .
