Title : Accident During Repairing a Rack.

On 20 September 2015 at 3.30 pm, an accident happened at maintenance workplace during repairing a rack. This accident involved a worker which is Ali. It happened
during Ali grinding the welding spot at rack to flatten the surface. He did't realised that he not wearing a pair of goggles which caused his eyes injured because was exposed to iron dust and spark from grinder machine. He was taken to the nearby clinic by his supervisor, Kamal for the treatment. In the future, all staff should wear the PPE to avoid this accident happen again and any dangerous situation.

TITLE : ACCIDENT AT METAL DEPARTMENT

On 15th September 2015 at 2:00 a.m, the incident happened at metal department when Mr Amin used stamping machine. He lost his fingers while handling the stamping machine. As the time was 2:00 a.m, he was sleepy, worked more than ten hours directly without rest and didn't focus on his job during the incident happened. The other workers that saw the incident immediately pushed the emergency button, released the hydraulic pressure and pulled out Mr Amin's fingers that stuck in the machine. The local area first aided was treated Mr Amin and then transported to hospital by ambulance. In the future, the workers should not work more than ten hours directly without rest and they should manage their rest time wisely.

Title: ACCIDENT AT PLASTIC DEPARTMENT

On 16th october at 5.00 p.m at plastic department, a worker had fell to the floor

and caused his head bled. The caused of this accident are the floor is slippery

caused of oil and that worker don't wear safety boots. The worker wanted to moved

from one section to another section by walked. A witnesses saw the worker was on the

phone and don't looked at his path properly.

Because of this accident, the worker bled his head badly. I as the person in charge,

I immediately gave him first aid action to slow down the bled on his head and I

called the ambulance and arrived 10 minutes later and the

worker is taken to the hospital for further action. Recommendations for prevent this

accident to happen are all workers must attend "safety at a workplace"

training. Beside that, the housekeeping system must always be organize. Last but not

least, the worker must be alert about any hazards that may be occur in a workplace

and wear proper PPE.

Title:Accident at Department Laser.


On 22 September 2015 at 3.15 p.m the incident worker at Department Laser Cutting.The workers did 5S' and walked at the machines Laser area.An the door from Laser machines lost balance and fell under he shoulder and the shoulder is bloated.The injured was then treated by the panel Clinic.Doctor give he two days MC.From this incident,I would like to suggest the maintenance can do the preventive maintenance and always check the safety at the machines always in good before start run the machines.

Title : Maintenance Report Accident

Date : 21th September 2015
Day : Monday
Time : 11.00 a.m (shift A)
Location : G/L 5
Victim : En. Firdaus
Accident Type : Finger pinched at V belt


The Supervisor Mr. Rama had told En. Firdaus the maintenance to change the belt and pulley size C177 at the G/L 5 as requested by production line leader En. Amin. The belt at G/L 5 was wear and tear and the pulley was stuck and need to change its bearing. After disassembling the pulley from the motor, En. Firdaus and his team works, En. Ahmad put away the pulley away form the motor. En. Firdaus change the bearing while En. Ahmad replace the belt. During the installation of the new belt to the pulley, En. Firdaus accidentally stuck his finger between the belt and pulley, En. Firdaus's finger broken and bled. He scream in pain and his team works quickly rotate the pulley anti-clockwise to safe the En. Firdaus's finger from being lost, and stop the bled with bandage form first aid kits. En. Ahmad then told Mr. Rama about what happened to En. Firdaus. Mr. Rama then called the ambulance. The ambulance then took En. Firdaus to the hospital. Mr. Rama then suggest to the safety department to train all the maintenance about the important of safety equipment and safe working methods during work in the future.

Title:Safety first
Date:9th june 2010
Time:10:35 am,

at the office,one of my operators Mr.Daus wiring struck electric shock when he handled the plug socket.Mr Ali enjoined Mr Daus to repair plug socket.Mr Daus struck electric shock because he not focus and not used the safety boots and Personal Protection Equipment (PPE).To control the situation Mr Ali has closed the main fuse to Mr Daus safe from electric shock.Mr Abu witness the incidents and he tried to help Mr Daus from electric shock.Mr Abu and Mr Ali provide help by diverting Mr.Daus from the plug socket using a broom stick.Recommendations to prevent such accidents happening is to wear proper safety especially PPE and focus on the job.

Title : Conveyor Belt Accident

Bakri had switched off the machine and was conducting routine checks on the conveyor belt. He saw that one of the connections was no good. He started to change it when Badrol switched the machine on. The conveyor belt moved Bakri's left hand was caught in it. His hand was seriously injured. There were no witnesses to the accident. However, Busu, who was working nearby, heard Bakri's yell and run for his aid. Seeing Bakri's injury, Busu run to get the first aid kit. Meanwhile, Badrol called the supervisor, Bahar. Bakri was taken to Bahar's office. Busu wrapped the injured hand using bandage and Bahar called the ambulance. Recommendations to prevent similar occurrences had be made. First, a safety procedures should be conducted strictly. Second, details of person to contact in the event of emergency should be displayed.
