On 5 August 2015 at 9.30 am Kamal got injured while working in warehouse. Kamal was hit by forklift. He goes to warehouse to take item and he walk on forklift lane. The forklift drive by faiz. He careless and drove in dangerous way.Kamal get a broken legs.Faiz inform to the supervisor and then kamal transported to the hospital by ambulance.
On 4 August , 2015 at 11.17 a.m , the incident happened at oil store of maintenance department. Mat the store keeper filled up the forklift oil. After filling , he did not carefully closed the oil tank and accidentally bumped the tank and the oil was spilled on the floor. Suddenly Shafie walked through the spilled area with big package which blocked his sight and fell down injured his backbone. The injury treated by our technician ,Mahmud with minor aid like putting pillow behind Shafie's neck and body for temporary relief while waiting for ambulance that Mahmud called for Shafie to be transported to the hospital. After the injured bought to the hospital , the cleaner wipe and clean the floor.
On 7 July 2014 at 10 a.m there was injury at store equipment under maintenance department.He was being handled cutter careless and caused injury bleeding to his finger.When work, he was cut the cable with cutter.It happen because he not wear glove to cut the cable during his work.As supervisor, I bring first aid to Kugen and give him a first treatment.I clean up his blood bleeding and wrap his bleeding using bandage.Lastly, I make a called ambulance for further treatment.
On 3rd August 2015 at 3.15pm, Deneshwaran was walking to washroom at warehouse. As he was rushing, he didn't used the walkpath, instead he walked through the loading bay as shortcut where the forklifts were working. One of the forklift hit him. Deneshwaran was seriously injured on his left knee. He was rested at the restroom while waiting for the ambulance to transport him to the hospital later.
