Title:Safety Issue On 7th Sep 2011 at 10.30 am, at the module production floor, one of my operators who is Rajendra met an incident while operating his cutting process. The incident happened because he didn't wear a proper PPE. Because of that, he got a serious injury on his finger caused by sharp knife. I transported him to hospital for medical treatment. For future reference, I suggest that we provide our operators a proper training about safety issue and we have to make sure our sharp items have been verified by the quality and safety before using it.

The supervisor Mr.Nazim had told Mr.Ang the technician to repair the CNC Drilling machine at the CNC Cylinder Head Machining Line as requested by production operator.The machine motor was broken and need to change its bearing.After dissambling the motor from the machine,Mr.Ang and his colleague,Ah Jun put away the motor away from the machine.Mr.Ang then opened the motor and changed the bearing.He removed the old bearing and put on shelve near by the machine.During the installation of the new bearing to the motor, Mr Ang accidentally bump into the table where he put the old bearing. The old bearing then fell down onto his feet.He screams in pain.His colleague quickly remove the bearing from Mr.Ang feet.Ah Jun then told Mr.Nazim about what happened to Mr.Ang. Mr.Nazim then called the ambulance.The ambulance then took Mr.Ang to the hospital.Mr.Nazim then suggested to the safety department to train all the technician about the important of safety equipment during work.

TITLE:An accident On 10 Aug 2015 at 3:00PM on the site have an work accident.The worker fell from the fifth floor.The worker work at fifth floor with no safety measures.The worker must be put on the PPE when he working.Another worker at the site saw the incident.A side supervisor immediately called police.The sided supervisor must conduct a safety seminars and put a safety board to reminded all worker.
Title:Safety first™ On 9th june 2010 at 10:35 am,at the office,one of my operators Mr.Dili wiring struck electric shock when he handled the plug socket.Mr Jujie enjoined Mr Dili to repair plug socket.Mr dili struck electrick shock because he not focues and not used the safety boots.To control the situation Mr Jujie has closed the main fuse to Mr Dili safe from electric shock.Mr Ang witness the incidens and he tried to help Mr Dili from electric shock.Mr Ang and Mr Jujie provide help by diverting Mr dili from the plug socket using a broom stick.Recommendations to prevent such accidents happening is to wear proper safety and focus on the job.
Title : WORK PLACE ISSUE On 20th sept 2015 at 2:00p.m on the building that named A company had an accident that involved SHAHMI . The accident that happened is SHAHMI got hit by sharp object . And other worker that saw the accident immediately call for a help. SHAHMI is being rushed to a nearby hospital . SHAHMI is okay but his leg is full of blood because the accident. It happened because the work place is unorganized and because the worker did not focus while doing their job . In the future, I hope that A company will train all the new workers about 5S and safety of work place. I also hope that the safety will become an important thing at all company.
